چکیده
This study is to identity the clinical characteristics of mydriasis derived from nebulized ipratropium bromide in neurosurgical patients. We presented 3 patients with inhalation of ipratropium bromide associated mydriasis and reviewed the pertinent literature. Mydriasis was caused by nebulized ipratropium bromide leak from the mask into ipsilateral eye. When the medication was discontinued in the 3 patients, the pupillary abnormality was resolved in 2 patients, but 1 patient’s pupil was not recovered. Ipratropium bromide blocks muscarinic acetylcholine receptors in sphincter pupillae. It absorbs through the cornea and conjunctiva, blocking the sphincter pupillae cholinergic M receptor antagonist lead to mydriasis, if lasts for a long time it will compress anterior chamber angle, impede real water reflux, cause increased intraocular pressure, induce glaucoma and ocular pain. It should quickly identify mydriasis reason in the clinical work, avoiding unnecessary invasive operation and imaging examination, to reduce the burden of patients.

Nebulised ipratropium causing a unilateral fixed dilated pupil in the critically ill patient: a report of two cases.
We describe two cases of a unilateral fixed dilated pupil secondary to the ocular instillation of nebulised ipratropium bromide. In one patient there was no other neurological abnormality. The second patient was unconscious following a cardiac arrest. While a fixed dilated pupil is an alarming sign, if it is caused by ocular instillation of ipratropium bromide the condition will resolve, althou...
متن کاملAnisocoria with high dose ipratropium bromide inhaler.
I bromide is an atropine derived anti-cholinergic bronchodilator used in obstructive lung diseases. Anisocoria mainly caused by the direct effect of nebulized ipratropium bromide via a leaking mask has been well described in the literature.1,2 However, anisocoria induced by systemic absorption after long-term use of high dose ipratropium inhaler is not well reported. We report a case of a 24-ye...

A 61-year-old woman is admitted to the ICU with head trauma following an alcohol-induced fall down a flight of stairs. She is known to have moderately severe COPD, but is otherwise in relatively good health. Her admission CT scan of the head is normal. Five days after ICU admission, she has a Glasgow coma scale score of 10, with pupils 4 mm and reactive to light. She opens her eyes to voice, lo...
متن کاملTransient anisocoria in a patient treated with nebulized ipratropium bromide
Purpose We present a case report of a 44-year old female patient with complicated pneumonia who developed anisocoria after treatment with inhaled ipratropium bromide. Discontinuation of ipratropium bromide treatment led to complete resolution of anisocoria. Observation A 44-year old female patient was admitted to the Department of Pulmonology due to high body temperature (40.0 °C), coughing a...
